“BEST” test - The game changer of non-invasive cardiac imaging
Abstract
Cardiovascular disease remains the leading cause of death worldwide, where early diagnosis and timely management are crucial for favourable outcomes. Two-dimensional echocardiography (2D Echo) is the most widely used non-invasive diagnostic tool; however, it has several limitations, including inter-operator & inter-subject variability, reliance on subjective interpretation, assumptions in volumetric calculations and lack of accessible to advanced tools or training. Although high-end echocardiography machines and specialized software can provide highly accurate, validated and real-time rendered images- they remain unaffordable for many healthcare centers in developing countries, limiting their role in patient care.
To address these gaps, we have developed and used an indigenous, open-source, cost-free tool over the last 15 years: Blender 3D Software in Cardiology, collectively termed as “BEST Test”. The acronym represents: B = B-mode (Brightness), E = Echo / Enhanced, S = Simulations, T = Transformations. This offline approach extracts clinically significant cardiac parameters from conventional 2D Echo, provides customizable, reproducible and comprehensive reports at zero cost.
Key unique features include: realistically rendered high-definition images, frame-independent analysis (useful in tachycardia), myocardial-focused assessment, Repeat M-mode, both 2D and 3D Left Ventricular volume evaluation, Endocardial imaging, Bull’s-eye mapping, Speckle Tracking, Valvular assessment and Audiovisual representations of heart sounds and murmurs. Future expansions may include 3D Printing, Artificial Intelligence-based mass reporting, Holographic visualization and Virtual Reality integration. Currently, Doppler analysis is not incorporated.
We do not claim the BEST Test is superior to existing techniques, but propose it as an additional, offline, multipurpose tool. Beyond echocardiography, it may also be applied to chest X-rays, cardiac CT scans, and invasive or diagnostic catheterization procedures. Validation studies comparing it with established evidence-based methods are ongoing. Patent processing and a dedicated global-access website are also in progress. The scope of this innovation is limited only by imagination, artistic application, and evolving software capabilities.
Biography
Dr.Chandra Shekara Reddy has completed his MBBS in 1996 from Mysuru University, Karnataka, India and Clinical Cardiology in 2008 from IGNOU (Indira Gandhi National Open University) from New Delhi, India. Published over 10 National and International Journals. Served in Medical department for about 5 years and 24 years in Cardiology in Tertiary Care Hospitals. Being first one to take care of patient with Mechanical Ventilator support for unknown reason at home for more than two months.
Pioneered and conducted 3 times a conference called “CRITICON” - a special conference on Critical Care mainly involving Nursing staffs, which was widely accepted during 2006 - 2009. Actively participated in all academic activities. Actively involved in more than 20 National and International Clinical Trials. Also involved in Research activities with special interest in 2D Echocardiography. During 2023 - 24 conducted an investigator initiated “BEST Test” project, sponsored by Apollo Research Institute (ARI). As an introductory - first time in the world the topic, “BEST” Test - The Game Changer of Non-Invasive Cardiac imaging has been published in Heart India Journal.
